Introduction to Social Connector for Social and Lifestyle Interventions

This programme is customized to equip staff of Active Ageing Centres (AACs) and Active Ageing Centres (Care) [AACs (Care)] with the foundational knowledge and skills to connect between primary care and the community; and help seniors to follow up on their Health Plans. The aim is to empower senior to stay active and manage their own health goals to their best abilities.

Social Prescribing is an important new intervention to improve the social determinants of health, advocated by World Health Organisation (WHO) and health systems around the world. This course builds on Social Prescribing, Social Determinants of Health (SDOH), Patient Centred Care and Biopsychosocial Model as cornerstones to improve the integration of health and social care. In this course, participants will learn and acquire skills to use tools to assess SDOH, find assets in the community to support patients and co-develop personalised care plans.

Motivational Interviewing for Social Prescribing

Motivational Interviewing (MI) is an evidence-based approach to behaviour change in healthcare, social work and many other settings. It is a client-centred and collaborative communication style. MI works by activating clients’ personal motivation and commitment towards better health, by exploring their own reasons for changes in lifestyle or adhering to prescribed treatment, in an atmosphere of compassion and acceptance.
